The narrator picks 1-3 Mafia members depending on how many people play. Probably only 1
The narrator picks 0-2 cops
The narrator picks 0-1 Doctors
All of these choosings is done in secret. Here it would be PMing
Everyone else is townspeople
Townspeople go to sleep
In the night the Mafia Members get to kill 1 person of their choosing
In the night the Cops get to ask the narrator if someone is a Mafia member. If they get it right, they have the advantage of knowing. Nothing else
In the night the Doctor can choose one person to save. If the mafia chose to kill that one person, then that person survives, but if they save someone who wasn't attacked nothing happens
Townspeople wake up, and the narrator announces what's gone on in the middle of the night.
The townspeople can vote on one person they want to execute
If they guessed the right person, then the narrator says the town is safer
Repeat until game ends by all mafia dying or all townspeople dying.

Needs at least 5 people, so it may not work.
